Please select your requested divisions. Teams may be placed higher or lower depending on the roster and/or past season results. If you are unsure which division to select, please choose unknown and we will contact you to try and find the best fit. Reminder teams can always be moved divisions after a few games into the season if the division is the wrong fit.

Payment Option

Teams can elect for a team fee option of $1900 (tax incl.) for up to 16 players. This option is paid in two installments by the team captain only, and it is their responsibility to collect fees from their players. Alternatively, teams can elect for individual payments at a cost of $165 (tax incl.) where individual players are responsible for their own payments to the league. This requires the players to pay their fee up front at the time of registration in order to play. If a team has 11 paid players, the team captain (or player of your choosing) may play for free.

Please list any other teams that your team shares players with to avoid the same divisional placement (if possible) and to avoid scheduling conflicts (we will do our best).

Please note any days (& times) of the week that do not work for your team and/or are preferred by your team. Ballhockey.com will do its best to accommodate whatever we can within reason for your team.

Team Roster

Please list your roster as you currently know it. This is not a definitive roster, but is used to provide the league with an idea of your roster for divisional placement and concerns. If your roster remains the same as it normally does, you can simply state "Same As Usual."

Registration Agreement / Liability Waiver

Upon submission, team representatives will be automatically registered to their respective team. If the team fee option is chosen, they will have a balance of $1900. If the individual fee option is chosen, they will be registered free of charge, unless they fail to meet the roster minimum by the roster deadline. Ballhockey.com Inc. strongly recommends that all participants wear a helmet, gloves, elbow pads, jock/jill strap and shin pads and all adult players accept that they are in control of and responsible for their equipment choices. All participants under the age of 18 must wear a CSA approved helmet with a full face mask, gloves and shin pads. I hereby grant permission to use and publish any photographs taken during ballhockey events of ME/MY child, or in which I/THEY may be included for editorial trade, advertising and/or any purpose and in any manner and medium: and to alter the same without restriction. I hereby consent to subscribe to emails from Ballhockey.com HOLD HARMLESS I, the applicant and WE/I the parent(s) of the applicant (if under 18) hereby give MY/OUR approval to MY/HIS/HER participation in any and all league activities during the current season. I/WE assume all risks and hazards incidental to such participation, including any Pandemic risks, transportation to and from the activities and I/WE hereby waive, release, absolve indemnity and agree to hold harmless the supervisors, officials, sponsors, organizers, and corporate owners of the premises. Also, persons transporting ME/OUR/MY child to or from activities for any claim arising out of an injury to ME/OUR/MY child.
